Shivam Chetan Chokshi, 21, of Hainesville, IL, and DePaul University of Chicago student passed away suddenly July 25, 2018 at the Northwestern Memorial Hospital, he was born January 25, 1997 in Arlington Heights, IL to the union of Chetan and Rita Chokshi, He was a younger brother to Kajal Chokshi, grandson to Shantilal and Padmaben Shah and late Ambalal and Sumitraben Chokshi, loved by many aunts, uncles, and cousins.
